The organization has developed a system for collecting, verifying and evaluated the credentials. All medical professional’s education, registration, training and experience is compiled and filed in the personal file.
The education, registration training and experience of all doctors and nurse are documented and filed in their personal file. These are maintained by the HR department. The following documents shall be collected from the doctors.
- Degree certificate
- Medical council of India registration certificate
- Experience certificate.
- Other certificates like participation in conferences, paper presentations etc. as applicable.
These files shall be checked once every year for completeness.
The information given by the medical and nursing professional shall be verified by the HR department.
Credentialing and privileging of nurses and doctors are done only by the authorized staff. The same will be counter checked by the HR manager and it is presented during the Credentialing and privileging committee which will be approved or revised by the committee chairman
The education, registration, training and experience of the medical professionals is documented and updated every year. It is also updated as and when new skills and qualification is acquired.
The educational qualification, training and experience of the medical professional are verified at the time of joining. Any subsequent skill and qualification acquired, is also included in the personal file after scrutinizing in the relevant records.
Medical professionals are granted privileges to admit and care for patients in consonance with their qualification, training, experience and registration.
The organisation shall identify services which each medical professional is authorized to do through credentialing and privileging with the help of a structured form.
C&P is first done by the department HOD. This is approved by the credentialing and privileging committee chairman. This shall be done based on qualification, experience and any additional training received.
The requisite services to be provided by the medical professionals are known to them as well as various departments / units of organization through internal communication mail.
Acknowledgement signature is taken from the employee and a copy of the same is filed in the concerned department.
Newly recruited staff will be kept under the induction period for the first week. During the period, induction from the HR department and department induction by the concerned department HOD will be given. Based on the skill and knowledge level of the employee the HOD will decide whether the employee is capable to perform independently the privileges granted. Medical professionals are authorized to provide only those privileges which are approved by the Credentialing and privileging committee
The services provided by the medical professional are in tune with their qualification, training and registration. The organisation maintains relevant records to this effect.

The training undergone during working in this hospital is also compiled and the relevant records are updated to include new skills and qualification. Job responsibilities and skill charts are updated and are available in the HR Department and also with the Nursing Superintendent / Manager. The clinical work assigned to the nursing staff is in line with the qualification, training and registration. These are also enhanced from time to time by providing training of new skills
The information available in the personal files of the nursing staff is verified at appropriate levels by suitable methods and any lack of fulfilling the requirements is converted into training and its effectiveness is monitored.
The organisation ensures that the services provided by the nursing staff are in accordance with the Indian Nursing Council Act 1947 and other existing laws and regulations. This is monitored by the Head of the Department and proper records are maintained.
